A Personal, Portable Summary: for Us and the Person We Support (Online)
Create a simple summary with the consent of the person you support that can be used for health appointments. 1-hour, free information session for mental health carers. Do you have to repeat the same details about the person you support every time you meet a new clinician or service provider? A personal and portable summary can make your life easier. In this free session, you'll learn how to create a simple, adaptable document to keep track of important information—like medical history, medications, key contacts and support needs—so you don’t have to start from scratch every time. Consent is a critical part of this prior to sharing. You’ll walk away with: a ready-to-use portable summary template tips on what to include and how to keep it updated a practical tool to reduce stress and ensure accurate information is shared.
