Health professionals, family members, friends and other supporters can use this form to refer an unpaid mental health carer (not a paid support or peer worker) to Arafmi.
Before submitting this form, please make sure the carer has agreed to share their details with us.
We will contact the carer, or you if needed, within 2 working days.
